Massage Request Form

This form must be received at least two days in advance of the requested date. We typically cannot complete requests more than three weeks prior to your requested date. Our Point Wellness Team will respond to your request via email within three (3) business days. Submitting the form below is only a request, not a confirmed reservation. Treatment times and prices are subject to changes.
